Before making an appointment for the Frisco Public Library Acceptance Facility, please confirm that at least one of the following statments below are true.

I am applying for my first US passport.
I am applying with my child who is under age 16.
  • Both parents are required to be present when applying with minors age 15 and under. If one of the parents is not able to make it to the appointment, they must complete and notarize a Statement of Consent for Issuing a Passport to a Child (form DS-3053 is available on travel.state.gov) and bring a front and back copy of that parent’s ID.
My previous U.S. passport was issued when I was under age 16.
My previous U.S. passport was lost, stolen, or damaged.
My previous U.S. passport was issued more than 15 years ago.

If none of the above statements apply to you, you may be eligible to Renew by Mail using Form DS-82.
