By scheduling an exam, I agree to the following Student Guidelines and Requirements:

• I must bring a valid physical photo ID.
• Arriving on time is my responsibility. Anyone arriving 30 or more minutes late must reschedule.
• Non-test takers, including unsupervised children, are not allowed in the testing center.
• Guests are only permitted if needed due to a disability. please inform staff in advance. Note: if you need a personal assistant due to a disability, please work with Disability Services for approval prior to your appointment.
• Unauthorized items such as cell phones, hats, watches, food, and drinks must be stored in provided lockers.
• I will comply with all staff instructions and seating assignments.
• Scratch paper and pencils will be provided and must be returned to the proctor for shredding after the exam.
• If permitted, calculators must be inspected and cleared of memory. hand-held calculators are not allowed for TSIA2 and Accuplacer Math.
• Incidents of cheating will be reported to the Professor, Dean, or Testing Company.
